Grim Day is a tier 5 scythe which can be obtained through the Mezuvian Tower, and The Underworld at nightmare difficulty. Like all tier 5 equipment, they all have 3 perks and between 8 to 10 upgrades available.

The weapon is dropped after killing the tower boss, and it has a extremely low chance of drop (1.6% to 2.5%). The level the weapon has is linked to your current level, so if you were to be level 94, it would have that same level. As for the nightmare dungeon, this drops as level 100 instead.
